• Please visit the web site for the National Labor Relations Board and under the "Forms" heading (http://www.nlrb.gov/forms) click on the NLRB Form 501 - Charge Against Employer.

• Your assignment is to complete TWO of these forms for any two of the cases discussed, or cited to, on pages 348 (starting with "The Duty to Disclose Information") to 357 of the text book. 

• You are free to use fictional information where necessary (e.g., contact information, number of workers employed, etc.).
